Step 2: Fill Out the Official Application Form

Step 3: Attach Required Documents

- Signed commitment letters from both the supervising pharmacist and the auxiliary staff.

Eligibility
- Must be a registered Professional Auxiliary Staff with a valid Database Identification Number (DIN).

Validity
