Losing your vehicle number plate or logbook can be stressful, but the Uganda Revenue Authority (URA) has simplified the process of getting a replacement. Whether your documents are lost, stolen, or damaged, you can apply for a duplicate number plate or logbook in Uganda either online or in person at any URA branch.
This process ensures that all vehicle ownership records remain valid and traceable under URA’s centralized motor vehicle registration system.
Before Applying
Before you begin your application, complete the following essential steps:
- Report to the Police: Visit the nearest police station and file a report about the lost or stolen number plate or logbook. You will be issued a police report.
- Advertise in a Newspaper: Publish an advertisement in a local newspaper about the lost item, typically within 14 days of the police report.
- Keep Copies: Retain copies of the police report and newspaper advert, as they must be attached to your application.
Option 1: Apply Online
You can apply for a duplicate registration book or number plate using the URA e-Services portal.
Steps:
- Visit the official URA website.
- Click on the “Login” button in the top-right corner and select “Portal.”
- Log in using your TIN and password.
- Under e-Services, select Motor Vehicle Registration → Application.
- Choose New Form, then select Alteration as the application type.
- Click on the Vehicle Details Page and fill in the required details.
- Attach scanned copies of your supporting documents as prompted.
- Proceed to the Payment Registration Page, then click on Register Payment.
- Review the displayed alteration fee details and make payment using your preferred mode (bank or mobile money).
- Print out your acknowledgment receipt, form, and payment slip.
- Submit these printed documents and attachments to your nearest URA licensing office for verification and processing.
Note:
In cases of lost, stolen, or damaged number plates, you may be referred to an authorized number plate manufacturer to produce the replacement plate.
Option 2: Apply In-Person
If you prefer manual application, visit your nearest URA branch and follow these steps:
- Visit a URA Office: Go to the nearest URA branch or licensing office.
- Request an Assessment Form: Ask for the assessment form for a duplicate number plate or logbook.
- Complete the Form: Fill out the form accurately and attach all required documents.
- Make the Payment: The officer will issue a payment receipt. Take it to a designated bank, pay the applicable fees, and obtain the bank payment slip.
- Submit Application: Return to the URA office with your filled form, supporting documents, and bank receipt.
- Acknowledgment: The officer will verify your documents and issue an acknowledgment slip.
- Processing: Once processed, URA will notify you, and the duplicate number plate or logbook will be delivered or made available for collection.

For Duplicate Number Plate:
- Completed application form (stamped if non-individual).
- Police report (for lost or stolen plates).
- Damaged plate (if applicable).
- Newspaper advert for lost plates (optional).
- Laboratory report from CID headquarters (if both plates were lost – optional).
- Copy of the applicant’s identification card.
- Payment slip for duplicate fees.
For Duplicate Logbook:
- Completed application form (stamped if for a company).
- Damaged logbook (if available).
- Police report and newspaper advert (if lost).
- Copy of identification document.
- Payment slip for duplicate fees.
Eligibility
All motor vehicle owners registered in Uganda whose number plates or logbooks have been lost, stolen, or damaged are eligible to apply for a duplicate through the URA.

Processing Time
