In-Person Application Procedure

1. Visit the NSSF Office

2. Obtain and Complete the Application Form
   • Request the Invalidity Benefit application form from the NSSF customer service representatives. Carefully fill out the form with accurate details.
3. Prepare the Required Documents
   • Ensure that you have all the necessary documents for your application. The required documents include:
     • Disengagement letter, termination letter, end of contract letter, or introduction letter from your last employer or local council.
     • Medical certificate or clinical notes documenting the illness or injury (you may be required to see the NSSF-appointed doctor).
     • Certified High Court order (if applicable for persons with mental incapacity).
     • Valid ID (e.g., national ID, passport, driving permit).
     • Proof of bank account details (bank statement or slip).
4. Submit the Application Form
   • Submit your completed application form, along with the required documents and a bank slip showing proof of payment, to the NSSF office.
5. Processing and Approval
